Hi Madelyn, thank you for your response to the clinical case scenario. However, your response to the diagnosis of type 2 diabetes can also be looked at through other diagnostic lenses like the Fasting Blood Sugar test and Glucose Tolerance test apart from HbA1C and Random Blood sugar (CDC, 2023). Not only would the tests be simpler but also be quite prompt at diagnosing type 2 diabetes mellitus. Besides diagnosis, another major point that has been missed out in your post is the occurrence of two chronic comorbidities in the above case. Not only should we mention the generic diabetes management but also consider its comorbidities associated with COPD. Patients with COPD and Diabetes Mellitus tend to suffer an increased risk of pneumonia as well as respiratory failure (Gunasekaran et al., 2021). Based on the requirement of a 65 year old Male patient with DM and COPD whose quality of life is at stake, a comprehensive plan comprising the management of two co-morbid conditions should be made. Your point that he should quit smoking is valid. However, he should be advised with breathing exercises, and cardio-pulmonary physiotherapy apart from diet and lifestyle modifications (Asif, 2023). 

Complications of COPD and Diabetes Mellitus

Likewise, the complications of the 65 years old would be more severe if his condition goes mismanaged or not managed properly. Some of the complications arising from comorbid conditions are not just those associated with generic diabetes but also COPD and DM. For instance, the deadly complications arising out of the two conditions can be conditions like respiratory failure, acute kidney injury, and stroke apart from heart diseases, neuropathy, and affected eyes. So, there are a few points that should be taken together in consolidation for making a comprehensive comorbidities management plan in its entirety. This will not only improve the quality of life of the patient but will also reduce further complications. Thank you!
