Hello Katherine! Thank you for your post on the Patient-Centered Medical Home (PCMH), which is not a place per se but a collaboration of the primary care providers to provide a patient-focused healthcare (pcpcc.org, n.d.). The concept of PCMH is indeed a welcoming step to provide optimum patient care to the patients as it primarily focuses on the patient well-being as well as it is led by a team-based collaboration. On one hand, the patients experience quality care as the entire multidisciplinary team is focused on providing patients with preventative healthcare, patient participation and their education (Agency for the Healthcare Research and Quality, n.d.), the healthcare providers are also benefitted at the same time as it decreases a collaborative effort to decrease clinicians’ burnout (Alidina et al., 2014). Additionally, it streamlines processes as well as standardizes procedures to provide the staff with cost-effective and efficient tools to ensure success of the organization (National Committee for Quality Assurance, 2021). Thus, Patient-Centered Medical Home is an efficient model for both the healthcare providers and the patients.

Hello Desirae, Thank you for your write-up. I agree with you that the team-based healthcare model is beneficial for both the patients as well as the healthcare professionals in the sense that it not only amplifies the effort of the clinicians but also enhances patient’s well-being. The team-led healthcare providers work in synchrony towards the same goal of the well-being of the patient (Smith et al., 2018). Not only does it provide staff with cost-effective, and efficient, working model but also ensures patients with the quality care that they ought to get (Smith et al., 2018). APRNs can definitely promote this model by enhancing their participation in the political, social and professional decision-making (Mason et al., 2021). Thus, it is a model which is profitable, reduces clinicians’ burn-out and is focused on patient care by enhancing their well-being.
